What the Form Includes
The new patient form for children collects the following information:
- Child's personal and contact information
- Parent or guardian details
- Medical history and current medications
- Dental history and any concerns
- Allergies and sensitivities
- Emergency contact information
- Insurance and billing information
- Consent for treatment
What to Bring to the First Visit
- Alberta Health Care card
- Dental insurance card or policy information
- List of any medications your child is currently taking
- Any relevant medical records or referral letters
- Previous dental X-rays if available (can be sent digitally)
- Parent or guardian photo ID
How to Complete the Form
You can complete the new patient form in one of the following ways:
- In-office: Arrive 15 minutes early to complete the form at our front desk.
- By phone: Call us at (403) 291-4945 and we can take your information over the phone.
- Online booking: When you book online, you can provide preliminary information through our booking system.
